Having committed to serve in a Central Church's ministry and to the habits essential for spiritual maturity, I agree to continue the following:

I have read and commit to Central Church's 5 Points of Connection. (see below)
I will continue to prepare for ministry by growing in my personal relationship with Christ.
I will continue to commit to regularly attending Central Church’s weekend services - not just serving.
I will continue to support and pray for Central Church's ministries and the congregation I serve.
